For the past 15 years, the University of Wisconsin Population Health Institute, part of the University of Wisconsin School of Medicine and Public Health, has led the County Health Rankings & Roadmaps, with primary funding from the Robert Wood Johnson Foundation. This program provides data to help local communities improve health. The 2025 Annual Report, released in March, includes a Health Data Brief highlighting the most and least healthy counties in Mississippi. It also provides a Health Snapshot for all counties in the state, focusing on two key areas: population health and well-being, and community conditions.

Population Health and Well-being includes:

  • Length of Life
  • Quality of Life

Community Conditions includes:

  • Health Infrastructure
  • Physical Environment
  • Social and Economic Factors

Starting in 2024, County Health Rankings & Roadmaps (CHR&R) began using a new method to group counties based on overall health. They calculate a single “z-score” for each county and use it to sort counties into 10 groups (Health Groups 1-10). This makes it easier to compare a county to others with similar health conditions within the state, region, or across the country.

Selected Measures for 2025 University of Wisconsin Public Health Institute (UWPHI) Model of Health with weighting for Length of Life/Quality of Life and Health Infrastructure, Physical Environment and Social & Economic Factors.

The figure below indicates the rankings of the Mississippi Counties (82) -from Least Healthy in US to Healthiest in US. Following the map graphs are provided for each Mississippi County (2025 release) by Population Health and Well-Being and Community Conditions based on the previously mentioned different measures. Individual measure for each county can be obtained if desired from the full report at the University of Wisconsin Population Health Institute https://uwphi.pophealth.wisc.edu/
